Job Summary
Provide services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives under the direction of a case management supervisor. Obtain in-depth social histories and develop agency resources to meet consumer needs. Serve as the Quality Improvement and Data Provider (QIDP) and provide monthly monitoring of the program. Ensure compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. This position involves extensive travel and requires a valid driver's license and car insurance.
Required Qualifications
- Valid driver's license
- Required car insurance
- Bachelor's degree in a human services field
- One (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field
- Valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing
- One year of experience working directly with individuals with intellectual and/or developmental disabilities
- Bachelor's degree in a human services field
- Two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field
- Valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing
- Two years of experience working directly with individuals with intellectual and/or developmental disabilities
- Ability to obtain in-depth social histories
- Ability to develop and assure community and/or agency resources to meet the consumer's needs
- Ability to serve as QIDP and provide monthly monitoring of the program
- Ability to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines
