Job Summary
The Case Manager I is responsible for providing comprehensive case management services, including engagement, assessment, and intake of clients, developing individualized case plans, and offering ongoing support. This role requires collaboration with other specialists, conducting field visits, and facilitating connections to community resources. Candidates must possess a Bachelor's degree in a relevant field or an AA with two years’ relevant experience, along with experience working with vulnerable populations.
Required Qualifications
- Bachelor’s degree in social work, Human Services, Counseling, or related field or AA degree in the same with two years relevant experience
- Proven experience in case management or a related field
- Must be available to work in person 5 days a week
- Able to conduct home/field visits and travel throughout the five boroughs
- Ability to utilize public transportation and conduct physical activities such as climbing stairs and walking
