Job Summary
Coordinate client intake, complete assessments, and apply for Medi-Cal while connecting individuals to housing, employment, and community resources. Assist clients with resume writing, transport them as needed, and monitor progress toward treatment plan goals through collaboration with clinical and behavioral health teams. Submit progress notes within 72 hours, complete URCs, and prepare court reports to ensure compliance with agency policies and external standards. Deliver services across outpatient, residential, and field settings while meeting a weekly productivity expectation of 25 client care hours.
Required Qualifications
- Must complete background check and livescan clearance
- Option I: High school diploma or equivalent
- Option I: Substance Use Disorder Counselor registration from an accredited California agency (CCAPP, CAADE, CADTP)
- Reliable transportation
- Maintain full compliance with requirements for registration or certification
- Attend and actively participate in assigned Clinical Group Supervision
- Actively participate in agency and team meetings
- Participate in training opportunities and complete all assigned training in a timely manner
- Arrange work schedule in accordance with the agency's needs
- Read emails daily and respond when appropriate
