Job Summary
Case Manager I in an Emergency Men's Shelter coordinates enrollment and diversion services, develops Housing First Individual Service Plans, conducts housing assessments, coordinates support services (medical, mental health, dental, transportation), and advocates for clients to move from homelessness to housing. The role requires building rapport with clients, documenting progress notes, coordinating with Case Manager II, and delivering services in the shelter setting while maintaining confidentiality. Strong computer literacy, written and verbal communication, and the ability to operate in a culturally diverse environment are essential. Flexible scheduling to accommodate evenings and weekends is required.
Required Qualifications
- Prior experience with homeless population required.
- One year of full time professional level casework experience in a social service agency preferred.
