Job Summary
Conduct strengths-based assessments of housing barriers and develop individualized Housing Stability Plans for chronically homeless adults. Provide case management, advocacy, and coaching to increase income, secure employment, and connect clients to medical, mental health, and community resources. Coordinate services with staff and external partners to prevent eviction, manage crises, and ensure housing stability for residents at Drexel House and Hope Village. Track participant data in HMIS and Microsoft Teams while participating in multidisciplinary case staffing meetings. Assist with transportation to appointments and lead group educational series to support independent living skills.
Required Qualifications
- Post-Secondary education in social services
- 1-year experience providing direct services to low-income/no-income households in crisis
- 3 years' experience providing direct services to low-income/no-income households in crisis
- Detail oriented and can perform organizational duties with a high level of accuracy
- Demonstrate ability to manage and educate
- Demonstrate quality communication and listening skills
- Ability to support and maintain an environment that honors cultural awareness and competency
- Ability to support the missions set forth by each program and their parent organizations CCS/CHS as well as the mission of the Catholic Church
- Willingness to learn and work well in a team environment
- Must have reliable transportation
- Valid driver's license
- Automobile insurance
- Acceptable driving record per CCS' driving policy
- Proof of negative TB test within past 12 months
- Applicant must successfully pass required background checks prior to an offer of employment

Desired Qualifications
- Masters degree in a psychology, social work, or related field
- 3-5 years' experience with individuals experiencing homelessness
- Experience organizing schedules
