Job Summary
Case Manager I at Catholic Charities of Acadiana delivers trauma-informed case management and supportive counseling to individuals experiencing homelessness or instability, connecting clients to housing, behavioral health, and community resources. Responsibilities include conducting strengths-based assessments, developing individualized service plans with measurable goals, coordinating care across internal and external providers, maintaining accurate documentation, and providing crisis support and safety planning. Field-based visits to shelters, community settings, and client residences are involved, with emphasis on building stability, resilience, and connections to housing, health, and employment services. The role requires collaboration with multidisciplinary teams, adherence to confidentiality, and professional boundaries, as well as knowledge of local resources and housing systems and proficiency with electronic documentation and Microsoft Office. Bachelor’s degree (Master’s preferred) and at least 1 year of relevant experience are required/preferred.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or related field preferred
- Minimum 1 year of case management or social services experience required
- Experience in homelessness, behavioral health, or crisis environments strongly preferred
- Strong understanding of trauma-informed care and harm reduction principles preferred
- Excellent communication, organizational, and documentation skills
- Ability to build rapport and maintain professional boundaries with diverse populations
- Proficiency with electronic documentation systems and Microsoft Office Suite
- Knowledge of local community resources, public assistance programs, and housing systems
