Job Summary
Case Manager I at Behavioral Health Services Inc coordinates care for clients with behavioral health needs, conducting assessments, developing individualized care plans, advocating for clients within healthcare and community resources, coordinating services, maintaining documentation in compliance with regulatory standards, providing crisis intervention, educating clients and families, and participating in multidisciplinary team meetings to adapt care plans and improve outcomes.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field from an accredited institution
- Minimum of 1 year of experience in case management within a behavioral health setting
- Experience working with diverse populations and understanding of cultural competence in service delivery
- Prior experience in developing and implementing individualized care plans
- Strong organizational and communication skills, both verbal and written
- Proficiency in using case management software and maintaining accurate client records
- Ability to conduct thorough assessments and develop comprehensive care plans
- Strong problem-solving skills and ability to work independently and as part of a multidisciplinary team
- Confidentiality and adherence to ethical standards
- Familiarity with community resources and ability to facilitate referrals and coordinate services for clients
