Job Summary
Case Manager position within an interdisciplinary team supporting individuals in maintaining stable, independent living in the community. Responsibilities include home visits, counseling, crisis intervention, and service coordination to help consumers access medical, social, educational, vocational, and financial resources. Roles involve assessing needs, developing individualized service plans, maintaining case records, providing 1:1 and group counseling using cognitive‐behavioral techniques, coordinating services with other providers, assisting with benefits and resources, conducting crisis assessments, accompanying to appointments, safety inspections, planning activities, participating in supervision and required training, and advocating for consumer needs including discharge planning.
Required Qualifications
- Bachelor’s degree in a human services field
- Associate’s degree in a human services field and two years of full‐time direct service experience
- High school diploma/GED and four years of full-time human services experience
- Experience providing direct services to individuals with mental health needs, including counseling, case management, crisis intervention, and service coordination
- Certifications/Licenses: No specific license required
- Must be able to travel within the community; a valid driver’s license may be preferred depending on assignment
- Work Eligibility: Must be authorized to work in the U.S.
- Work Location Requirement: Onsite / Community‐based field work (regular travel to consumer residences)
