Job Summary
Assess, plan, implement, monitor, and evaluate client care cycles to meet health and human services needs under minimal supervision. Coordinate safe, timely, and client-centered care by maintaining regular contact, handling case assignments, and drafting service plans. Deliver ongoing services including crisis intervention, advocacy, medication monitoring, and housing assistance while developing individualized treatment plans and ensuring compliance with agency policies. Document all clinical activities accurately and participate in treatment teams to recommend client status changes. Provide client education and facilitate group curricula. For school-based or community assignments, deliver up to 30% of time on prevention services, including MEB health education, PBIS collaboration, and screening for student needs to connect individuals with resources. Complete required training and adhere to professional standards.
Required Qualifications
