Job Summary

Engage households transitioning out of homelessness with intensive case management, including independent living skills, housing stabilization, and crisis intervention. Collaborate with clients to create biopsychosocial assessments and goal plans, while maintaining accurate client files and adhering to agency documentation standards. Provide trauma-informed support for medical, mental health, and substance use barriers, and advocate for clients to address housing and eviction challenges. Travel regularly across Los Angeles County to connect participants with resources, attend case conferences, and facilitate tenant events at project-based housing sites. Work a 9/80 schedule with flexible evening and weekend availability, requiring a valid driver's license and personal vehicle for field duties.

Required Qualifications

  • Bilingual proficiency in Spanish/English
  • Proficient in Microsoft Programs (Word, Excel, Access, PowerPoint), Zoom, and web-based scheduling software
  • Ability to work a 9/80 work schedule
  • Ability to work a flexible work schedule, including evenings and weekends as needed
  • Obtain and maintain CPR/First Aid Certification
  • Ability to pass post-offer Tuberculosis (TB) clearances
  • Regular and prompt attendance in the office is required at all times
  • Travel is a regular duty for this position and is required
  • Drives personal vehicle in and around Los Angeles County or drives agency vehicles periodically to transport clients, worksites and other locations is required
  • Must have and maintain a valid California Driver's License and auto insurance in good standing
  • Maintain and execute confidential information according to HIPAA standards
  • Respond in a timely manner in all aspects of communication with team members, participants, and partners
  • At least one (1) year of work history and relevant transferrable skills

Desired Qualifications

  • Familiarity with best practices in homeless services, such as Housing First, Harm Reduction, Motivational Interviewing, Strengths-Based Case Management, and Trauma-Informed Care is preferred
