Job Summary
Case Manager- Housing Stability within a Certified Community Behavioral Health Center (CCBHC) model. Responsibilities include engaging clients, coordinating and advocating for access to housing and social services, completing clinical documentation, coordinating care with medical and behavioral health providers, and participating in multidisciplinary teams. Requires attending team meetings, maintaining client records, conducting wrap-plan development and updates, and ensuring safety planning and crisis intervention. Emphasis on trauma-informed, recovery-oriented, and client-driven care for adults experiencing homelessness or housing instability with serious mental illness or co-occurring disorders. Must hold or obtain Case Management Certification and a valid driver license.
Required Qualifications
- Bachelor’s Degree in the social sciences preferred (consideration for Criminal Justice or Education).
- Case Management Certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire.
- Valid Driver License and satisfactory driving record; use of personal automobile to travel to locations other than primary office.
- Strong commitment to client rights and community-based living.
- Experience or ability to work with adults experiencing homelessness or housing instability with serious mental illness, SUD, or co-morbid conditions.
- Ability to document client progress and maintain permanent records.
- Familiarity with CCBHC standards and integrated team-based care.
- Ability to coordinate care across health services and engage family members in treatment planning.
