Job Summary
Support individuals with mental illness and co-occurring disorders as a primary support and case manager, assisting them to live independently in their chosen community under direct supervision. Provide housing specialist services to help clients maintain stability while navigating community integration, transportation, and benefits. Drive clients to appointments and groups using either a personal vehicle or company vehicle as needed. This role is part of a 100-member Assertive Community Treatment (ACT) program in Santa Clara County dedicated to adults and older adults with severe and persistent mental illness, co-occurring substance use issues, and complex needs.
Required Qualifications
- High School Graduation or G.E.D. equivalent
- three (3) years of direct services in mental health or community services
- Associate's degree
- two (2) years of experience
- Bachelor's degree in Social Services
- at least 1 year of direct service experience providing assistance as a Housing Specialist
- Valid and current driver's license
- personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc.
