Job Summary
Case Manager for the Housing Advocacy & Resource Team (HART) supporting individuals and families experiencing homelessness to locate, obtain, and maintain stable housing. Build strong relationships, manage an active caseload, conduct assessments and develop housing-focused goal plans, coordinate care across medical/mental health/substance use resources, collaborate with landlords, address eviction risks, educate on tenant rights and independent living skills, document services accurately, and work with teammates and community partners to support client success. Requires a high school diploma or GED (or an associate degree or higher) and a valid driver’s license with ability to travel within the community. 32-hour full-time work week with full-time pay and benefits including health plan, HSA, dental, FSA, Life/AD&D, long-term disability, 401(k), paid holidays and generous PTO. Required Qualifications
- High school diploma or GED
- One or more years of experience working with individuals experiencing homelessness or similar populations OR an associate degree or higher (no experience required)
- Valid driver’s license, reliable transportation, and ability to travel within the community
