Job Summary
Case Manager focused on housing stability for individuals and families experiencing homelessness or housing crisis. Responsibilities include assessing housing environment, developing housing and success plans, coordinating with landlords and program partners, facilitating referrals and resource connections, crisis intervention, and monitoring eligibility and enrollment documents. Role involves community-based work, frequent client meetings in clients' homes, and driving to serve clients. Strong written communication, knowledge of housing resources, and ability to coordinate eviction-prevention and subsidy activities are required; candidate should maintain confidentiality and work within trauma-informed, strength-based practices.
Required Qualifications
- High School Diploma required
- Bachelor's degree in social work or related field preferred
- Minimum of 2 years of experience in case management required
- Must possess a valid driver's license and own transportation
