Job Summary
CASE MANAGER for transitional housing: manage a caseload of 25 clients, perform initial assessments within 48 hours, develop Independent Living Plans, and coordinate referrals for medical, mental health, legal, education, and housing services. Deliver comprehensive case management including crisis intervention, counseling, and support, using evidence-based practices (EBP, Motivational Interviewing, Trauma-Informed Care, CTI) and maintain CARES data entry and client charts. Facilitate client orientations, establish housing and employment goals, track progress, and ensure compliance with contractual requirements. Duties include facilitating intake documentation, facilitating referrals, preparing mandated reports, assisting with income/public benefits, and coordinating with housing coordinators to support permanent housing. Requires evenings and one Saturday per month, with 35 hours weekly (3 FTE). Qualifications include a BS in social work or related field, 1 year of experience, and English/Spanish proficiency as a plus.
Required Qualifications
- Bachelor's degree in social work or a related field is required
- One (1) year of social service experience or approved equivalent
- Familiarity with E.B.P., strength-based case management, motivational interviewing, trauma-informed care, CTI, boundary setting, and cultural reflexivity preferred
- Bilingual – English/Spanish proficiency a plus
