Job Summary
Join PATH as a Case Manager on the Interim Housing team to assist clients in breaking the cycle of homelessness through individualized supportive services. Responsibilities include conducting intake assessments, developing Individualized Service Plans (ISP), providing referrals, maintaining client documentation, and utilizing motivational interviewing. Candidates should demonstrate strong analytical skills, communication, teamwork, and problem-solving abilities. The role requires a commitment to client confidentiality and proactive engagement in service delivery.
Required Qualifications
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Successfully complete Tuberculosis Test
- Background Screening
- Drug Test
- Have reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
