Job Summary
Coordinate with local MCPS staff and contact families within 24 hours of program referral acceptance. Assess individual client needs and collaborate on developing specific service plans, including initial case management goals and objectives. Link families to medical, social, educational, vocational, and recreational services while monitoring progress and documenting outcomes. Facilitate family team meetings with MDHS, review Crisis Management Plans, and provide direct case intervention services in home, school, and community settings. Ensure all services are reflected in documentation and complete weekly ISP reviews and program reports. Act as a positive role model and provide transportation for families as required.
Required Qualifications
- Bachelor's degree required in Social Work or a related field
- at least two (2) years' experience working with children and families
- Demonstrate competence in working with children and families in the child welfare system
- Demonstrates competence in completing documentation and reports related to services provided
- Computer skills required
- Must be at least 21 years of age
- have a valid driver license
- a good driving record
- proof of legally appropriate auto liability insurance
- must be willing to use personal vehicle for organization business
