Job Summary
Conduct pre-screens, intakes, and assessments to determine participant eligibility and appropriate assistance levels for low-income families at risk of homelessness. Engage families through strength-based case management, developing service plans and goals to ensure stable, permanent housing. Act as an advocate by assisting with tenant rights education, landlord mediation, legal service connections, and crisis interventions. Connect clients to supportive services, budget counseling, and self-sufficiency programs while tracking monthly goals and objectives. Document program activity, including financial assistance and case management, into HMIS within one to three business days. Submit monthly narrative and data reports as required. Transport clients to appointments as needed and participate in agency program quality improvement activities.
Required Qualifications
- Two years of experience working in outreach, shelter, or supportive housing programs for homeless or at-risk households
- Bachelor's Degree or equivalent combination of education
- Current driver's license
- Access to a vehicle
- Personal automobile insurance
- Current CPR and First Aid Certification
- Fingerprinting/background check
- TB test
- Ability to work with basic mathematical concepts
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
- Knowledge of Internet software
- Knowledge of Spreadsheet software
- Knowledge of Word Processing software
- Regular attendance

- Bilingual
- Personal or lived experience with housing instability, social services, systemic challenges, or the criminal/legal system
- Internet research skills
