Case Manager- Homeless Outreach

Job Summary

Coordinate care for adults experiencing homelessness with serious mental illness and substance use disorders by planning, advocating, and linking clients to medical, social, legal, and educational resources. Document client progress, participate in daily organizational and treatment team meetings, and complete initial wrap plans and intakes as needed. Provide crisis intervention, monitor health status, and accompany individuals to medical appointments while ensuring timely transfers during hospitalizations. Facilitate access to housing, employment, and community services to support recovery within a strengths-based, culturally sensitive framework.

Required Qualifications

  • Must possess a valid Driver License and satisfactory driving record
  • Must have a strong commitment to the right and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services
  • Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues
  • respect for client rights and personal preferences in treatment
  • Case Management Certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire

Desired Qualifications

  • Bachelor's Degree in the social sciences field
