Job Summary
Coordinate with service teams to develop independent living plans for clients experiencing homelessness and meet regularly to help them execute these plans and secure housing. Provide direction and referrals for employment, education, medical, mental health, and childcare services based on individual client needs. Identify causes of homelessness and maintain multiple priorities while keeping stakeholders informed of case status.
Required Qualifications
- Bachelor's degree in social work or a related field OR equivalent experience and skills
- Ability to keep track of multiple priorities, completing tasks on time and keeping the appropriate people informed of the status
- Great communication skills (written and oral)
- Want to help people experiencing homelessness
