Job Summary
Case Manager in CAMBA's Health Homes program responsible for reviewing client eligibility documentation, assisting with intake applications, creating and maintaining client files, carrying a caseload of ~50 clients, conducting initial assessments, coordinating case conferences, following up with referrals, advocating for client self-sufficiency, monitoring progress toward service plan goals via calls and home/office visits, documenting outcomes, reporting as required, liaising with outside organizations on education, healthcare, housing and entitlements, escorting clients to appointments, and potentially planning social/peer events and inputting progress data.
Required Qualifications
- Associate’s degree and 4 years applicable experience OR Bachelor’s degree (B.A. or B.S.W.) and two years of applicable experience
- Bi-lingual English with Spanish or Haitian-Creole
