Case Manager - HCHV Residential Housing Program

Job Summary
Conduct intake interviews, psychosocial assessments, and service planning for Veterans admitted to the Health Care for Homeless Veterans Residential Housing Program. Develop Individual Service Plans within one week of admission and update them every 30 days. Meet with assigned Veterans at least twice monthly, perform suicide and safety screenings, and coordinate therapeutic interventions for complex medical, behavioral health, and substance use challenges. Support housing placement by assisting with applications, paperwork, and securing permanent housing opportunities. Facilitate structured groups, activities, and therapeutic programming while collaborating with VA medical, behavioral health, and social work teams. Maintain accurate case records in HMIS and ensure compliance with HIPAA and Federal standards. Full-time onsite Monday through Friday between 8:00 AM and 8:00 PM.
Required Qualifications
- Master's Degree in Social Work, Mental Health Counseling, Human Services, Psychology, or related behavioral health field
- Current unrestricted New York license as Licensed Master Social Worker (LMSW)
- Current unrestricted New York license as Licensed Mental Health Counselor (LMHC)
- Minimum two (2) years of experience providing case management or behavioral health services
- Experience serving individuals experiencing homelessness and/or populations with chronic medical, mental health, or substance use conditions
- Experience conducting psychosocial assessments and developing treatment/service plans
- Ability to assess and anticipate crises and respond appropriately
- Ability to perform office and residential program duties including standing, walking, bending, and occasional lifting up to 20 pounds
- Availability onsite Monday through Friday between 8:00 AM and 8:00 PM
Desired Qualifications
- Experience serving Veteran populations
- Experience with residential treatment or contract residential care settings
- Experience with HMIS
- Experience using Housing First, Harm Reduction, trauma-informed care, and motivational interviewing approaches
