Job Summary
Lead psychoeducational and process groups, plan and facilitate group sessions, and provide client engagement within a structured, supportive environment. Conduct case management tasks including resource coordination, goal setting, discharge planning, and serve as a liaison between clients, clinical staff, and external providers. Collaborate with a multidisciplinary team to align group content with treatment plans, participate in team meetings, and contribute to treatment progress reviews. Maintain accurate documentation of group notes and case management updates while ensuring confidentiality and regulatory compliance. Support program development to enhance group curriculum and community resource integration. Requires travel occasionally during the workday and occasional overnight stays; schedule is Monday–Friday, 8:30 AM–5:00 PM.
Required Qualifications
- High School Diploma with Substance Use Disorder (SUD) Registration Certification OR Bachelor’s Degree in Psychology, Social Work, or related field
- Minimum one (1) year of experience facilitating behavioral health or mental health groups
- Valid Driver’s License with clean driving record
