Job Summary
Provide case management for tenants in supportive housing; conduct intake and assessments to determine program eligibility; develop and update person-centered support plans; coordinate care with social services team and collateral contacts; facilitate tenant engagement in employment, education, volunteering, and life-skills development; provide targeted interventions to prevent eviction; advocate for tenants’ access to medical, mental health, and community resources; maintain tenant files with accurate documentation; participate in trainings and collaborate with property management and community providers to support tenant independence and quality of life.
Required Qualifications
- Bachelor’s degree in social work, mental health counseling, psychology, sociology, or related field, with 3-5 years’ experience in a residential setting
