Job Summary
Case Manager - Gilroy, CA on-call PART-TIME position overseeing mentor/companion care within a case management program. Responsibilities include conducting weekly/bi-weekly/monthly sessions with the client, maintaining consistent communication with the client’s team (family, clinical and non-clinical providers, financial guarantors), developing and revising client reintegration plans, providing phone support, documenting all interactions, managing toxicology screens and medication management with mentors/companions, and facilitating pre-approval for out-of-pocket reimbursements. Must have a valid driver’s license and be willing to transport clients as needed. Hours may fluctuate based on client demand and coverage needs. This is a W-2, on-call/as-needed role.
