Job Summary
Conduct comprehensive assessments and develop individualized service plans for families seeking housing stability and self-sufficiency. Provide ongoing participant-centered case management through regular engagement and monthly contacts, coordinating housing stabilization efforts and facilitating referrals to community partners. Develop, implement, and update person-centered plans that reflect changing participant needs, employment goals, and long-term self-sufficiency objectives while maintaining accurate documentation for compliance. Build and maintain relationships with stakeholders to ensure access to resources for healthcare, housing, education, and financial planning. Collaborate with FCRHA support staff and participate in team meetings to align on grant objectives and progress toward program goals.
Required Qualifications
- Bachelor's degree in social work, psychology, or human services
- Valid driver's license and/or access to reliable transportation to perform work-related travel
- Eligible drivers must have a good motor vehicle record (MVR)
- Annual criminal background checks, child abuse clearances, and other required screenings in compliance with ServiceSource, state, and funding agency regulations
- Availability and willingness to travel weekly within the local area to perform the job
- Availability and willingness to work flexible hours, including evenings and/or weekends, when needed
- Proficiency in Microsoft Office products
- Strong verbal and written communication skills
- Excellent organizational skills
- Detail-oriented and able to carry out work with the highest levels of accuracy
- Committed to social inclusion model and strengths-based approach
- Knowledge of continuum of care and transition planning
- Ability to establish and maintain positive working relationships with participant systems
- Knowledge of public benefits (SNAP, TANF, Medicaid, Medicaid Waivers, and Medicare, etc.)
- Ability to walk up and down flights of stairs
Desired Qualifications
- Master's degree
- 2-5 years of experience working with vulnerable populations, including households receiving TANF or living in subsidized housing, and/or homeless, immigrants, or justice-involved populations
- 2-5 years of experience in case management, care coordination, and the development and implementation of individualized plans
- Experience working with participants receiving Fairfax County housing subsidies, including HCV, PBV, public housing, Mainstream Vouchers, EHV, and other FCRHAâadministered federally assisted programs
