Job Summary
Case Manager (GBHI) responsible for intensive case management and supportive services to individuals experiencing homelessness with mental health and/or co-occurring substance use disorders. Responsibilities include conducting comprehensive assessments across behavioral health, housing, medical, financial, vocational, and social service needs; developing individualized care plans with SMART goals; coordinating housing resources (emergency shelter, permanent supportive housing, and community-based supports); assisting clients with identification documents, benefits enrollment, medical insurance, and employment resources; coordinating care with behavioral health and medical providers, probation/parole officers, shelters, and other agencies; providing client advocacy to reduce barriers to treatment and housing instability; conducting outreach, crisis intervention, and follow-up to support ongoing participation; ensuring compliant documentation per program standards, HIPAA, and 42 CFR Part 2; participating in multidisciplinary meetings and promoting trauma-informed, recovery-oriented approaches; ability to deliver linguistically and culturally appropriate services.
Required Qualifications
- Graduation from an accredited 4-year college with a degree in Psychology, Social Work, Community and Human Services, Rehabilitation Counseling, or a related field
- Most have a valid Guam Driver's License
- Experience in case management or related field preferred
- Ability to develop individualized care plans and coordinate services across providers
- Ability to coordinate housing resources and access to supportive housing and services
- Proficiency in culturally and linguistically appropriate service delivery (interpreter use as needed)
- Knowledge of HIPAA and 42 CFR Part 2 regulatory requirements
- Strong documentation and staff collaboration skills
