Case Manager - Full-Time

Case Manager for a mental health residential program in Jamaica, Queens, NY. Responsibilities include coordinating and monitoring assessment, treatment planning, and care coordination for each resident; advocating for residents’ rights; helping residents achieve goals toward independent housing; linking residents to community resources and services; ensuring medical and dental care; maintaining coordination with financial specialists and treatment providers; participating in interdisciplinary team meetings; developing service plans and monitoring goals; providing crisis management when needed; ability to sit/stand and lift as required; requires a valid driver’s license.
