Job Summary
Case Manager coordinates and monitors assessment, treatment planning and care coordination for residents in the Mental Health Apartment Treatment Program. Serves as advocate for residents’ rights and helps develop person-centered Service Plans. Connects residents with community resources such as employment, day programs, vocational rehabilitation and health care services; ensures periodic medical and dental care; collaborates with the interdisciplinary team to integrate care between the residential program and other services; may conduct skill-building classes and support residents toward maximum autonomy and independent housing. Requires valid driver's license and ability to meet physical demands; duties include crisis management, visiting clients, and supporting program goals within a disability-conscious, nondiscriminatory framework.
Required Qualifications
- Valid Driver License Required
- Bachelor’s degree or Associate’s degree plus one year experience or High School diploma plus two years’ experience in mental health setting
