Job Summary
Case Manager collaborates with the physician to provide individualized program management for each patient, ensuring progression through the continuum of care and achieving desired clinical and financial outcomes. Oversees clinical and financial coordination of the treatment plan for assigned patients, ensuring timely, cost-effective, and individualized service delivery. Works with rehabilitation patients with diverse disabilities (e.g., spinal cord injury, brain injury, cerebrovascular accident, amputation, neurologic disorders, orthopedic conditions, arthritis) and coordinates length of stay management within CMS guidelines and a 60% compliance threshold.
Required Qualifications
- Current state licensure as a Registered Nurse, Licensed Social Worker, PT, OT, SLP
- Minimum of 3 years healthcare and clinical experience in a hospital setting
