Case Manager - Full Time - EMP, SFV

Job Summary

Maintain a caseload of program participants seeking supported employment through the Department of Rehabilitation, Regional Center, and Social Security. Liaise between clients, job sites, and the IPP team to create contacts with potential employers and spearhead the Discovery process. Conduct client assessments, refine resumes, and facilitate classes on budgeting and benefits while overseeing job coaches and program staff. Process referrals and documents, attend case review meetings, and prepare progress reports for regulatory audits. Represent TASC on community boards and committees to enhance local knowledge and foster team engagement.

Required Qualifications

  • 2 years of experience working with adults with developmental disabilities in a vocational setting
  • Bachelor's degree or equivalent experience in social work or related fields
  • Desire to compassionately align with the client, while supporting a culture of creating self-reliance and sustainability
  • Ability to establish and develop effective relationships with staff, employers, volunteers, community groups, current and potential donors, corporate sponsors, agency partners and other key stakeholders
  • Strong computer skills, including demonstrated success with Microsoft Office Suite
  • Ability to work in multiple locations (office/remote/community)
  • Must be able to successfully complete First Aid/CPR, ACRE, Core Competency Training, Direct Support Professional Training, and other trainings as required
  • Must have current California driver's license and clean DMV
  • Must have reliable transportation and current auto insurance
  • Must receive criminal record clearance through the DOJ
  • Ability to drive for work
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards

  • Ability to occasionally lift and/or move up to 50 pounds
  • Ability to provide physical assistance and/or push clients up to 250lbs in a wheelchair
  • Ability to assist adult clients transitioning in and out of vehicle and in the restroom
  • Completion of Professional Crisis Management Certification within 90 days of hire
