Job Summary
Case Manager position at Episcopal Community Services (ECS) focused on direct, in-person client engagement with residents experiencing homelessness, behavioral health conditions, or related challenges. Responsibilities include intake assessments, regular in-person outreach, development of individualized Service Plans, referrals to internal/external resources, benefits eligibility work, budgeting assistance, advocacy with property management to maintain housing, conflict resolution, Wellbeing Checks, facilitation of weekly therapeutic/psycho-educational groups, thorough documentation, discharge planning, program development, back-up coverage for team members, information sharing with residents and staff, and adherence to ECS policies. Required skills include strong written and verbal communication, documentation proficiency, crisis de-escalation, group facilitation, cultural competency, and the ability to work in a multi-disciplinary team. Bilingual Spanish skills are desirable and may carry a 5% premium for bilingual roles.
Required Qualifications
- High School Diploma + 4 years’ experience or AA degree + 3 years’ experience or CHW Certificate/CADC + 2–4 years’ experience or BA/BS + 2–4 years’ experience or MA/MS degree + 1 year experience
- Senior Case Manager: MA/MS + at least 2 years’ experience, or BA/BS + at least 4 years’ experience, CHW Certificate/CADC + 4 years’ experience
