Job Summary
Case Manager manages a caseload of approximately 35 participants, conducts assessments, and develops Individual Service Plans focused on housing. The role involves implementing service plans with participants, coordinating housing, income, primary care, substance-use treatment, behavioral health, legal, and other resources; reporting progress to program leadership; maintaining accurate participant files in electronic systems; assisting with planned exits and linking to appropriate services; and ensuring compliance with agency, state, and funder regulations through collaboration with program leadership. The position is fully onsite and client-facing at ECS program locations, with emphasis on safety, de-escalation, trainings, and adherence to procedures.
Required Qualifications
- High School Diploma + 4 years’ experience or AA degree in similar field + 3 years’ experience, or CHW Certificate/CADC + 2–4 years’ experience, or BA/BS degree in similar field + 2–4 years’ experience, or MA/MS degree in similar field + 1 year experience
- Senior Case Manager: MA/MS degree + at least 2 years’ experience, or BA/BS degree + at least 4 years’ experience, or CHW Certificate/CADC + 4 years’ experience
- Experience working in integrated service teams using person-centered harm-reduction and trauma-informed approaches
- Background working with unhoused individuals
- Effective communication skills (written and oral)
- Excellent organizational and time management skills
- Strong administrative and organizational skills
- Must possess excellent charting, recordkeeping, and professional writing skills
- Knowledge of Microsoft Office programs and experience documenting services into electronic record systems
- Must be able to problem solve and quickly assess/respond to a variety of situations, including medical emergencies, psychiatric emergencies, conflict resolution and de-escalation interventions, as well as other types of crises
- Operate with cultural humility in a diverse program and staff population
- Possess a knowledge of and sensitivity to adults with substance abuse, domestic violence, incarceration, HIV/AIDS, mental health including co-occurring disorders, and history of trauma
- Lived experienced preferred, but not necessary
- Must secure fingerprint image screening and annual TB screening
- Must be able to meet physical requirements of the position, which may include lifting up to 30 lbs
