Job Summary
Complete timely and accurate documentation in the Uniform Case Record, including referral intake packets, correspondences, entitlement documentation, and logbooks. Review all intake information for new referrals, perform initial screening assessments, and make recommendations regarding acceptance or denial. Conduct annual Catholic Charities Housing Office intake assessments for new tenants. Develop and implement Individual Service Plans (ISP), report outcomes every six months, and address monthly progress against specific housing and referral milestones. No subject-to-call shifts required. Located in Albany or Troy.
Required Qualifications
- Associate's Degree in Social Work
- 1–3 years' experience with relevant experience in Human Services or a related field
- Valid Driver's License
