Job Summary
Coordinate interdisciplinary care for a designated patient population by reviewing inpatient admission records to ensure proper resource utilization and appropriate level of care determination. Assess and identify discharge needs while collaborating with the team to facilitate desired financial and quality outcomes. Maintain and enhance payor relationships to support patient populations across acute, IRF, or LTACH settings. Requires licensure in the state of residence, three to five years of inpatient experience, and current BLS certification. Full-time days schedule available at PAM Health locations in Pennsylvania.
Required Qualifications
- Current BLS certification
- Three (3) to five (5) years of inpatient experience
- Licensure in the state where the hospital resides is preferred
- Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker
Desired Qualifications
- Experience, preferably in an acute, IRF, or LTACH setting
