Job Summary
Case Manager provides aging services support to older adults and LGBTQ+ clients through person-centered, strengths-based case management, intake assessments, home visits, and referrals. Responsibilities include developing service plans, coordinating resources, advocating with healthcare providers and service systems, documenting interactions, and maintaining caseloads (approximately 60–75 clients). The role involves in-person work at Friendly House in Portland, OR, with home visits throughout the service area, occasional travel, and alignment with contract/program requirements. Requires two years of relevant experience or a related degree, valid Oregon driver’s license, ability to pass background checks, and strong communication and data-management skills. Union-eligible and represented by ILWU Local 5; schedule primarily Monday–Friday with possible evening/weekend work.
Required Qualifications
- Two years of experience working with older adults; or
- A degree in social work, gerontology, human services, or a related field; or
- An equivalent combination of education and relevant experience, especially with older adults and/or LGBTQ+ community members.
- Ability to successfully pass a criminal background check and motor vehicle record (MVR) check.
- Possession of a valid Oregon driver's license and proof of insurability required.
- Reliable, regular access to a personal vehicle sufficient to conduct home visits and community-based work throughout the service area, including areas not served by public transportation.
- Knowledge of the strengths, needs, and challenges experienced by older adults, including LGBTQ+ older adults.
- Familiarity with community resources and service systems serving older adults and/or LGBTQ+ community members.
- Understanding of professional boundaries and the ability to establish, communicate, and maintain appropriate boundaries with clients and in the community.
- Demonstrated initiative, critical thinking, and ability to work independently.
- Proficiency with computers and the ability to quickly learn new software and database systems.
- Excellent verbal and written communication skills, including in-person, telephone, virtual, and email communication.
- Working knowledge of strengths-based case management practices and person-centered service delivery.
- Ability to prioritize competing demands and shift between time-sensitive and ongoing projects with flexibility and professionalism.
- Strong problem-solving skills and the ability to work effectively in a fast-paced environment.
- Demonstrated reliability, punctuality, organizational skills, and effective time management.
- Experience working effectively with individuals from diverse social, economic, and cultural backgrounds.
- Preferred: Experience using ADRC CareTool, Multnomah County UCR, Oregon ACCESS, or similar data collection and case management systems.
