Job Summary
Conduct initial and ongoing assessments to evaluate clients' mental health, substance use, strengths, and needs. Develop personalized recovery plans in collaboration with clients, outlining specific goals and interventions. Coordinate with healthcare providers, social services agencies, and community resources to ensure comprehensive support. Advocate for clients' rights and preferences within healthcare and social service systems while providing emotional support and encouragement. Educate clients about mental health, addiction issues, treatment options, coping strategies, and relapse prevention. Teach life skills such as budgeting, time management, and communication to foster independence. Document all interactions, services provided, and outcomes accurately and timely. Work closely with multidisciplinary teams including psychiatrists, therapists, nurses, and social workers to ensure integrated care. Adhere to ethical standards and legal regulations, including confidentiality, privacy laws, 42/CFR Part 2, and HIPAA. Maintain accurate records and ensure compliance with documentation requirements.
