Job Summary
Case Manager for Vigo County in Indiana. Full-time hybrid role requiring travel to community-based meetings multiple times per week and remote work from a home office. Responsibilities include providing information and education about the Medicaid waiver process, creating individualized support plans based on client strengths and goals, helping Individuals access needed services, and facilitating regular community-based team meetings with service providers. Must have a Bachelor’s Degree in Human Services and at least one year of relevant experience; must possess a valid driver’s license, car insurance, and reliable transportation. Requires in-person meetings and local live within Indiana (or within 10 miles of the state line). Required Qualifications
- Bachelor’s Degree in Human Services
