Case Manager for Shelby County

Job Summary
Provide information and education to help Individuals understand the Medicaid waiver process; create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve; help individuals access services and supports they need to meet their goals; facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed. Requires a full-time, hybrid work arrangement with travel to community-based meetings multiple times per week, and working remotely from a home office. Applicants must live within Indiana or within 10 miles of the state line, with a designated home office setup. A valid driver's license and reliable transportation are required. A Bachelor’s Degree in Human Services and at least one year of professional experience working with Individuals with intellectual and developmental disabilities are requested. IPMG provides sign-on bonus and opportunities for advancement, along with expense and mileage reimbursement.
