Case Manager for Porter County - H&W/TBI Waiver

Job Summary
Case Manager in Indiana for Health & Wellness (H&W) and Traumatic Brain Injury (TBI) Medicaid Waivers. Full-time hybrid role with remote work from a home office and travel up to one hour for community-based meetings several times a week; candidates must reside within Indiana or within 10 miles of the state line. Provide information and education to help Individuals understand the Medicaid waiver process; create individual support plans based on strengths, needs, goals, and desires; help Individuals access services and supports to meet their goals; facilitate regular community-based team meetings to review plans of care and collaborate with service providers as needed; work with Individuals on H&W and TBI Medicaid Waivers. Requires a valid driver’s license, car insurance, and reliable transportation; education requirements include RN or bachelor’s degree in relevant fields or 2 years direct service experience or master’s degree in related fields. Employee ownership via ESOP and a sign-on bonus of $500 within first 6 months. Home-office setup with computer, internet, and smartphone required.
Required Qualifications
- A valid driver's license, car insurance, and reliable transportation
- RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
- The ability to pass thorough background screens
- Designation home office setting with computer, internet service, and smart phone
