Job Summary
Full-time hybrid Case Manager in Indiana (Indianapolis area) responsible for guiding Individuals through the Medicaid waiver process, creating Individual support plans based on strengths and goals, helping access needed services, and facilitating regular community-based team meetings with service providers. Requires travel to meetings, a valid driver's license with insurance, and a designated home office. Must have a Bachelor’s Degree in Human Services and at least one year of professional experience working with Individuals with intellectual and developmental disabilities.
Required Qualifications
