Job Summary
Case Manager position in Indiana ( Vernon, IN area) offering full-time, hybrid work with remote duties from a home office and in-person travel up to one hour for community-based meetings several times a week. Responsibilities include providing information about the Medicaid H&W/TBI waiver process, creating individualized support plans based on each Individual’s strengths and goals, helping access needed services, and facilitating regular community-based team meetings with service providers. Requires a valid driver’s license, car insurance, and reliable transportation. Qualifications may include RN or bachelor’s degree in social work, psychology, counseling, gerontology, nursing, or health & human services; or any field with 2 years of direct service experience with the elderly or disabled; or a master’s degree in related fields. Candidates must be able to pass thorough background screens and have a designated home office setup (computer, internet, smartphone). This role is designed for individuals residing in Indiana or within 10 miles of the state line and involves travel for meetings while maintaining primarily a remote/home-based work arrangement.
Required Qualifications
- A valid driver's license, car insurance, and reliable transportation
- RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
- The ability to pass thorough background screens
- Designated home office setting with computer, internet service, and smart phone
