Job Summary
Hybrid full-time Case Manager responsible for educating Individuals about the Medicaid waiver process, creating individual support plans based on the strengths, needs, goals, and desires of the Individuals we serve, helping Individuals access services to meet their goals, and facilitating regular community-based team meetings to collaborate with service providers; requires RN or bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in related fields, plus a valid driver’s license and reliable transportation; position based in Noblesville, IN with travel up to one hour for local meetings and hybrid work arrangement.
Required Qualifications
- RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
- Valid driver’s license, car insurance, and reliable transportation
- Designated home office setting with computer, internet service, and smart phone
- Ability to pass thorough background screens
