Job Summary
Case Manager position in Grant County (Marion, IN) offering full-time, hybrid work with remote home-office duties and travel up to 1 hour for in-person community-based meetings several times per week. Responsibilities include educating individuals about Medicaid waiver processes, creating individualized support plans, helping them access needed services, and coordinating regular community-based team meetings with service providers.
