Job Summary
Case Manager for Gibson County—H&W/TBI Waiver: Provide information and education to help Individuals understand the Medicaid waiver process; create an individual support plan based on the Individuals’ strengths, needs, goals, and desires; help Individuals access services and supports to meet their goals; facilitate regular community-based team meetings to review the plan of care and collaborate with service providers as needed; work with Individuals on Health & Wellness (H&W) and Traumatic Brain Injury (TBI) Medicaid Waivers; requires a valid driver’s license, car insurance, reliable transportation, and residence within Indiana or within 10 miles of the state line; hybrid work arrangement with remote home office and travel up to one hour for in-person meetings several times a week.
Required Qualifications
- RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
- Valid driver’s license
- Car insurance
- Reliable transportation
- Ability to pass thorough background screens
- Designated home office setting with computer, internet service, and smartphone
