Case Manager for Dubois County

Job Summary
Provides information and education to help Individuals understand the Medicaid waiver process, creates an Individual support plan based on strengths, needs, goals, and desires, and helps Individuals access services to meet their goals. Facilitates regular community-based team meetings to review plans of care and collaborate with service providers as needed. Requires a Bachelor’s Degree in Human Services and at least one year of professional experience with Individuals with intellectual and developmental disabilities; a valid driver’s license, car insurance, and reliable transportation; and a designated home office. This full-time hybrid role involves travel to community-based meetings multiple times per week and requires living in Indiana (or within 10 miles of the state line) with in-person meetings and remote work from a home office.
Required Qualifications
- Bachelor’s Degree in Human Services
- Minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities
- Valid driver’s license
- Car insurance
- Reliable transportation
- Designated home office setting with computer, internet service, and smartphone
Additional Requirements
- Must live within the State of Indiana or within 10 miles of the Indiana state line
