Job Summary
Case Manager at IPMG in Indiana manages Medicaid waivers for Individuals, develops individualized support plans based on strengths and goals, connects Individuals to needed services, and facilitates regular community-based team meetings with service providers. Hybrid role requiring remote work from a home office and travel for in-person meetings up to an hour away; candidates must reside in Indiana or within 10 miles of the state line. Requires a Bachelor's in Human Services and at least one year of relevant experience with Individuals with intellectual and developmental disabilities. Must have a valid driver's license, car insurance, reliable transportation, and a designated home office with internet and smartphone. Required Qualifications
