Case Manager for Behavioral Health

Job Summary
Case Manager provides comprehensive support and coordination of care throughout the addiction recovery process. Responsibilities include assessing treatment needs, developing and monitoring treatment plans, coordinating referrals to housing, employment, medical, and legal services, maintaining contact with community resources, coordinating post-discharge requirements, documenting progress in the EMR, facilitating interdisciplinary approaches, conducting group sessions, and communicating with families, courts, probation officers, and external agencies to ensure continuity of care. Requires thoughtful communication, knowledge of GMHSU integrated treatment, AHCCCS/RBHA/TRBHA systems, HIPAA and 42 CFR Part 2, and the ability to manage multiple cases (approximately 90 hours per month) while upholding ethical standards.
Required Qualifications
- Twenty-One Years or Older
- Minimum Education: High School Diploma or GED
- Meets Behavioral Health Technician (BHT) qualifications; 2 years related behavioral health experience or training or equivalent combination of education and experience
- High school diploma or equivalent required; associate’s degree or higher preferred
- CPR/First Aid Certification
- Nasal Naloxone Opioid Reversal Training
- Two-Step TB test clearance
- Must hold valid and active/unrestricted Arizona Driver’s License
- Background check and pre-employment drug screening
