Job Summary
Conduct intake interviews and determine needs for medical and psycho-social evaluations. Review therapist and physician evaluations, care objectives, and plans while coordinating services and advocating for entitlements. Discuss assessment findings with the physician and treatment team to maximize outcomes, assist in discharge planning, and coordinate after-care services. Initiate complete assessments to gather baseline data and identify barriers to post-inpatient care. Proactively work with patients, caregivers, and the interdisciplinary team to develop realistic discharge plans, including homecare or alternate level of care resources. Perform ongoing evaluations for facility level of care appropriateness in collaboration with insurance payers while complying with all State, Federal, and Joint Commission regulations.
Required Qualifications
- MA license required (RN, PT, OT, SLP, RT or LCSW/LICSW)
- Bachelor's Degree Related Field of Study
- Case management, utilization review, or discharge planning experience
- 2-3 years preferred
- Strong customer and interpersonal skills
- Knowledge of community service resources
- Proficient User in Microsoft Office Applications
- Strong commitment to clinical excellence
- Strong problem solving skills and team oriented behavior
- Strong patient/customer service skills
- Ability to verbally calm patients
