Job Summary
Case Manager Float at Duke Health responsible for managing an assigned caseload to ensure timely assessment, planning, implementation, and evaluation of discharge plans and care transitions across the continuum of care. Collaborates with interdisciplinary teams, patients, families, and community partners to coordinate safe, efficient care transitions; develops and evaluates individualized discharge plans; coordinates services (Home Health, DME, transportation, substance abuse treatment, SNF placement, Acute Rehab referrals); maintains documentation and supports payer negotiations and regulatory compliance; participates in QAPI activities and provides after-hours coverage as needed.
Required Qualifications
- BSN or MSW required
- Case Management Certification (ACM, CCM, or ANCC) required within 3 years of hire
- RN license in North Carolina (or compact) for BSN-prepared candidates
